Blackboard course request form

Request a New Blackboard Course

This request form is for any CFCC instructor who wishes to request a Blackboard course, either for development or "live" teaching. Please fill in all the fields listed below. All course requests must be submitted through this form.
We no longer combine multiple online or hybrid courses into one Bb course. For example, if you are teaching COM 110 IN1 and COM 110 IN2, you will submit this form twice, and have two Blackboard courses. This restriction does not apply to face-to-face courses using Blackboard as a supplement - those are allowed to be combined.
The creation process is not automatic, this form simply sends the required information to the Distance Learning Department Chair and the Blackboard Administrator. You will be notified by email when your course is ready for editing.

* Section ID		(Example: 'D01', or 'DEV' for development courses)
* Semester and Year		(Example: 'Spring 2008', or if this is just for development, enter 'development')
Course Title		(Example: 'ACC 121: Principles of Accounting II')
* Copy of Which Course	(enter "NEW" to start with just the CFCC template)	If this course is to be a copy of an existing course (re-use the assigments, documents, etc), list the course id here. (Example: XXX110-IN1-2006SP) Otherwise, the course will only contain template material. **NOTE: please don't just list a course title here, ie 'CHM 151', or 'General Chemistry', there may be several courses with that title. What we need is the course ID that uniquely identifies the course in Blackboard. When you are in Blackboard, go into the Control Panel for the course. At the top of the Control Panel, a blue title bar tells you the ID of the course, the title of the course, and the instructor name, for example `XXX110-IN1-2006SP: example course - my name (Instructor)`. If this was your course, you'd enter the ID 'XXX110-IN1-2006SP' in the box to the left.

Import VLC course?	(Yes if checked)	If you didn't ask for the course to be a copy above, do you want to use the VLC content, if available? Search available VLC courses (Just click Submit to search all courses.) What is the VLC?
Create test student user?	(Yes if checked)	If you already have a 'test' student account, please leave this unchecked, and if you'd like it to be enrolled in this courses, please remind us below.
